临床研究是探索和优化临床常见病诊疗技术和方法的有效措施。临床信息、生物样本和研究结果是支持临床研究的主要资源,临床电子化病历 (electronic medical record,EMR) 已经成为临床研究中不可缺少的信息资源。中国拥有丰富的疾病资源,电子化病历也积累了丰富的临床信息,但其在临床研究中的应用却微乎其微,主要原因可能是信息本身的可用性较低。临床研究中的受试者招募耗费大量经费和时间成本,充分利用EMR中的临床信息可有效利用招募资源。本文从电子化临床信息的现状出发,探讨其应用于临床研究的可用性、可行性及改善的可能性,阐述如何形成一套比较完善的流程、规范和方法,作为模式探索应用和推广,改善临床信息资源,建设临床研究库,推动临床和转化医学研究。

Clinical research is the powerful way we can study and learn how to prevent, diagnose, and treat illness. Clinical data, biobank, and research results are three major resources for clinical research. Subject recruitment is critical to the success of clinical research, but is costly in term of time and money. The increasing use of electronic medical records (EMR) has launched a discipline of clinical research, which offers great opportunities for clinical recruitment. China is rich in disease associated resources and has accumulated mountains of clinical data in EMR accordingly. However, the data in EMR are little used for clinical research. Here we review the challenging issues of EMR data in the hospital and discuss the possibility to improve quality and usability of EMR data for clinical research. This article also provides an overview of how to develop a strategic planning and methods to investigate if EMR linked associated biobank can be leveraged for clinical and translational research studies.
