肝纤维化是慢性肝病导致的一种常见的威胁生命的并发症,尽管其主要发病机制已被认识,但临床治疗仍然是个难题,早期诊断和干预极其重要。 肝组织活检虽然是金标准,但其具有创性和潜在并发症等缺点,使医生和患者不易接受,致无创诊断得以迅速发展。 目前肝纤维化无创诊断主要为生物标志物诊断和影像学诊断两个领域。 生物学指标由单一指标发展到多指标组合,多指标组合相对于单项生物学指标诊断准确性更高。 影像学诊断包括基于超声影像的瞬时弹性成像技术和声脉冲辐射力成像技术以及基于磁共振技术的磁共振弹性成像。 为更好地了解这些诊断指标和方法,为研究和临床应用提供帮助,特对这些指标和方法进行概述,并对其诊断价值以及其局限性进行探讨。

Liver fibrosis is the common life-threatening complication caused by chronic liver disease and hard to be cured,although its main mechanism has been known; its early diagnosis and intervention are extremely important. Liver biopsy,yet the “gold standard”,has not been accepted by doctors and patients because of a big risk of injury and potential complications and other defects.Therefore,increasing interest has been directed towards the identification of non-invasive tools in recent years. These non-invasive tools are mainly based on biomarkers and imageology. Beginning with a single index test,multiple-index combination tests have been proposed to improve the accuracy of the noninvasive diagnosis for liver fibrosis.Transientelastography,acoustic radiation force impulse and magnetic resonance elastography are main noninvasive diagnostic tools of imageology for liver fibrosis. In this paper,a brief introduction of these non-invasive methods is made to better understand them and provide help for research and clinical application;their diagnostic value and limitations are also discussed.
